After Billy the Beaut becomes the film's main villain, known as The Joker Jigsaw, his first act is to spring his brother from the mental asylum.

The brother is known as "Loony Bin Jim" for obvious reasons, and is seemingly in a catatonic state. We get the important point that he is extremely dangerous though, as he is strapped up in a rather silly spread-eagled pose.

He is attended to by an extremely overweight orderly,who resembles one of the "Boomer" zombies in Left 4 Dead.

He is also (as always seems to be the case in Hollywood films) not a particularly nice fellow.  At least he isn't a sadistic pervert like the ones in Terminator 2 or Kill Bill though, so Jim isn't in any danger of getting licked or raped.  No, this orderly's petty little moment of megalomania is to steal Jim's food instead of feeding it to him as he's supposed to.  Today he is yumming down "apple sauce", which is served baby food style with a plastic spoon.

Cut to a knock at the door and a demand from the duty doctor to be let in.

The greedy orderly reluctantly stops snacking and unlocks the door, whereupon Jigsaw strides in, snapping the neck of the doc as he does so.

There is a moment of fraternal affection as Jigsaw frees his brother and he then orders his accompanying toughs to kill the orderly.

"No, this one's mine!" says Jim, with a vengeful glint in his eye.

"So, you like applesauce do you?" he says to the cowering fatty. "Did you know kidneys and applesauce are a delicacy in Sweden? I'm gonna get my applesauce back!"

He is nothing if not true to his word either, and, as his brother looks on in mirth, he buries his face into the orderly's considerable torso and begins to savage it like a wild animal.  Just so we're not in any doubt what he's doing, we then see him lift out various entrails and, of course, a kidney.



Old Bluffer's Thoughts

This isn't a very sympathetic death, as nobody likes a fat, creepy mental asylum orderly - and he did steal the damn apple sauce!
