Mustafa actually did survive his fall in the second movie (Dr. Evil probably just assumed he had died when Mini-Me shot him, seeing as Mini-Me didn't stick around long enough to hear him shouting for help afterward). If you sit through the end credits, it cuts back to the cliffside, where Mustafa, still very much alive, only very badly injured, begins asking if anyone's up there, then begins calling out to the audience for assistance.

As a result, this does not paradox the present time, as it can probably be assumed someone found him and he survived, as a result he would still be alive in the 90's where Dr. Evil would successfully kill him.

I don't think Alotta Fagina survived, since the whole base exploded after Dr. Evil froze himself in again. But, of course, she could've been taken out by, who knows, Random Task, or Frau. Maybe Generalissimo, Jurgen, Rita and Don Luigi will return. Since Number Two actually got thrown in the burning basement too in the first movie, and eventually got out in the second movie (i'm still wondering how he did that).

Well, there was an escape hatch. Although Mustafa's death seems to indicate you can't open it from inside (for obvious reasons).

I always though Alotta died when Vanessa gave her a throat chop. It's a potentially fatal move in real life, as it punctures the windpipe.
