In the pre-credit sequence, we see 009 on the run, dressed as a clown, from a couple of flamboyantly dressed eastern European knife throwers. After a couple of near misses, he manages to get to the bridge border and climbing over the fence, he thinks he is home and dry.

However, Grishka or Mishka [it is impossible to tell one twin from the other] takes careful aim with his final knife and hits 009 squarely in the back. 009 falls into the river dead, or so we think.

[cut to Ambassador's residence]

The ambassador is holding a reception. Before the Ferrero Rochers can be handed out, a mortally wounded 009 comes crashing through the veranda doors, dropping dead at the ambassador's feet with a Faberge egg rolling from his grasp as he does so.

Shouldn't this be under "not quite dead" because of the Rasputin-esque ending?

Tough call. I wouldn't say so as the time from his getting stabbed to standing back up is pretty short, doesn't really allow "he's dead" to sink in.
