As always in action films, we reach the climactic ending where the hero (in this case Gibson) and the principal villain have to fight each other. This fight is set against the backdrop of a freight ship's cargo hold.
To add extra meaning to this contest, Riggs (and the audience) finds out earlier in this film that Vorstedt was responsible for the death of Riggs' wife, as well as the death of his current lover. After a very intense battle, Riggs manages to stab Pieter in the stomach with the knife that was buried in his leg. Riggs has the look of a madman on his face, and the defenceless Pieter fully expects to be killed.

However, Riggs releases his grip and hobbles off. Pieter, getting to his feet, pulls out a gun and points it at Riggs. What he doesn't know though is that Riggs has walked over to a remote control which operates the loading crane. Riggs pushes a button that releases a container which is hanging over Pieter. Before Pieter manages to get a shot off, he notices an ominous shadow fall over him. Looking up, he has just enough time to register surprise on his face before he is crushed by the container.

As a South African, I always find Pieter's death really funny, in that he's crushed in the end by a crate that bears the legend TRANSVAAL LINES on it.
I'm thinking Revenge, he had Riggs' wife and girlfriend killed.
