Rogue US Marshall, Deguerin, has just been indicted in a controversial arms-to-terrorists scandal. Upon leaving court, Kruger and the key witness get into a car which subsequently explodes. The audience are let into the secret that this is an "eraser" job when the camera focuses on a manhole cover sliding into place under the car.
Deguerin and his cronies (which include an undersecretary of defence) think that one of them have silenced Kruger and Lee (the witness) and they relax as their limo drives them away from the scene. As they discuss this favourable turn of events, they soon realise that none of them organised the car bomb and reality starts to set in. Suddenly, the limo doors are locked and the car comes to a halt on a railway crossing.
Deguerin then receives a call on his cell phone. Quizzically he answers it, only to hear Kruger utter the words "you have been erased". Deguerin turns around and looks out of the limo window, where he sees Kruger standing by the track, with an ominously advancing train passing him.
There is no doubt that the train could have stopped in time, but that would not be "Hollywood". Instead it ploughs on and through the limo, killing Deguerin and chums.
I have added the "Master / Pupil" category to this death, as much is made throughout the film about Deguerin being Kruger's mentor.
It is worth noting that minutes before the death (for the audience), there is a fight between the two atop a cargo container hanging from a crane, that would have qualified for "One on One", "Falling" and "Crushed by Falling Object".
And before that there is a skirmish aboard an airplane where Kruger "nails" him with a ("cheap hunk of metal") throwing knife, this qualifying for "Projectile" too.
However, in plot terms, these injuries were meted out quite some time before the final death, so it is fair enough that they aren't included. I only mention them to point out that the script writers were still playing the "death scene by numbers" game, or as Mouseburger is fond of saying, they were "ticking all the boxes".
Also, this film deserves a special mention for one of the weakest final lines:
Vanessa Williams: "What happened to them?"
Arnie: "They caught a train"
Admirers of the rather attractive Vanessa Williams in this film would probably enjoy doing a Google image search for her before she was famous...

Another mention goes to the song playing over the end credits - "Caught A Train".
You might want to make a page for Undersecretary of Defense Daniel Harper (Andy Romano). I can't confirm the 3rd passenger, so I'll get back on that.
Johnny Castelone (Robert Pastorelli) is technically the killer, as he parks the limo over the tracks.
A note might be made about the UK version - in the 15 certificate version, you don't see them get hit. When Robert looks out the window, it cuts to Krueger going to Cullen. Seriously.
The third guy was a fellow named Morehart. He worked in some capacity at the weapons lab.
Ah, him, played by Gerry Becker.
